Who Is Eligible to Use Form MV-904C?
Eligibility is limited. Personalized registration plates requested on MV-904C are restricted to passenger vehicles, trailers, or trucks with a registered gross weight or combination gross weight of not more than 14,000 pounds.
In addition:
- The vehicle must already have current Pennsylvania registration.
- New vehicles must be titled and registered before a personalized plate can be issued.

Applicants may list up to three choices in order of preference. When requesting the number zero, show it as “Ø” and mark any zero with a slash after printing the fillable form.
Always check availability first using PennDOT’s online tool on the Personalized Registration Plates page. Availability is not guaranteed; requests are processed first-come, first-served. A configuration is reserved only after PennDOT receives the completed application, payment, and approves the request.
Fees for Personalized Plates on Form MV-904C
A fee is required. The personalization fee covers only the cost of manufacturing and issuing the special plate; it does not renew the vehicle’s registration. If registration renewal is also needed, include the appropriate renewal form and separate payment.
Current personalization fees are listed on Form MV-70S, “Bureau of Motor Vehicles Schedule of Fees.” As of the most recent schedule:
- Standard-issue personalized plates: $103
- Most other (specialty/commercial) personalized plates: $136
