Who Needs to Use Form DL-54B?
Pennsylvania residents who already hold a Photo Identification Card use DL-54B when they need to:
- Replace a lost, stolen, damaged, or undelivered card.
- Renew an expiring or expired Photo ID.
- Update their address (a P.O. Box can be used in addition to a physical address but not alone).
- Add or remove an organ donor designation.
- Request removal of under-18 or under-21 banners after turning 18 or 21.

Name changes generally require an in-person visit to a Driver License Center with original supporting documents (marriage certificate, court order, or birth certificate). Gender designation changes require Form DL-32 in addition to DL-54B and must be processed in person.

How to Complete and Submit Form DL-54B?

Apply by Mail
- Download and complete Form DL-54B carefully. For replacements, complete Sections A, B, C, D (if applicable), E, and F. For renewals, changes, or corrections, complete Sections A, C, D (if applicable), E, and F.

If your photo is on file, you will receive a duplicate Photo ID with the existing image. If no photo is on file, a camera card will be mailed. You must then visit a Photo Center to have a new photo taken.

- Identification Card replacement: $42.50
- Identification Card renewal: $44.50

Special Notes for Applicants Under 21
If you have turned 18 or 21, you may request a replacement Photo ID without the “Under 18 until” or “Under 21 until” banner by using Form DL-54B (online, by mail, or in person) and paying the applicable duplicate fee.
Applicants under 18 must have a parent, guardian, person in loco parentis, or spouse age 18 or older complete Section D of the form and, in most cases, accompany them.
Important Tips for a Successful Application

- Sign the form in ink. For replacements, sign in the presence of a notary.
- Provide accurate information. Misstatement of fact is a misdemeanor of the third degree.
