Who Needs to File Form DL-80L1?
You need Form DL-80L1 if you currently hold a valid Ignition Interlock Limited License and any of the following apply:
- Your license was lost, stolen, or mutilated/damaged
- You never received your license (within 90 days of issuance)
- You need to change or correct your address, name, eye color, height, or organ donor designation
- You want to add the Veterans Designation
- You need a replacement camera card
Note: All name changes must be completed in person at a PennDOT Driver License Center with original supporting documents. Mail-in name changes via this form are not accepted.
How to Complete Form DL-80L1?

- Section A – Current License Information
Enter your full legal name, date of birth, current IILL number, expiration date, and a daytime telephone number (8:00 a.m. – 4:30 p.m.). - Check the Applicable Box
Select either “Replacement (Duplicate)” or “Change or Correction.” Complete the sections indicated for your choice. - Section B – Reason for Replacement (if applicable)

Pennsylvania licenses generally cannot be issued to out-of-state addresses except for specific federal/state government employees, armed forces personnel, and their immediate families. Supporting documentation is required. - Section C – Parental Consent (if under 18)
Must be completed and notarized if the applicant is a minor requesting organ donor designation changes. - Section D – Signature and Notarization
Sign in ink in the presence of a notary when required (for example, replacement of a camera card or certain free replacements). Affidavit language includes acknowledgment of Vehicle Code Section 3709 (littering) and a warning about false statements.
Optional contributions: You may add $3.00 for the Organ Donation Awareness Trust Fund or $5.00 for the Veterans’ Trust Fund.
