Ohio BMV 4531 Form: Free Disability Placard for Military/Veterans – Ohio Form BMV 4531 is the official “Application for Removable Windshield Placard for Active-Duty Military / Veterans with Disabilities.” Issued by the Ohio Bureau of Motor Vehicles (BMV), this form allows eligible active-duty service members and veterans with qualifying service-connected disabilities to receive a free (gratis) removable windshield placard.
The placard permits parking in spaces marked with the International Symbol of Access. Unlike the standard disability placard application (BMV 4826), Form BMV 4531 waives all fees for qualifying military personnel and veterans under Ohio Revised Code 4503.44.
Who Qualifies for a Free Placard Using BMV 4531?
Eligibility is limited to two groups:
- Veterans of the U.S. Armed Forces whose disability limits or impairs the ability to walk and is service-connected.
- Active-duty members of the Armed Forces (including Reserve components and National Guard) who have an illness or injury that limits or impairs the ability to walk.
The disability must meet at least one of the criteria in Ohio Revised Code 4503.44(A)(1), such as inability to walk 200 feet without resting, reliance on assistive devices, specific lung or cardiac conditions, severe arthritis/neurological/orthopedic limitations, or legal blindness.
Required Documents for Veterans
Veterans must submit:
- A completed BMV 4531 form (Sections A and B).
- A letter from the U.S. Department of Veterans Affairs dated within the past one (1) year confirming the disability is service-connected.
- Either a prescription from a qualified health care provider or Section C of the form completed by the provider.

How to Complete Form BMV 4531
Section A – Applicant information: full name, driver’s license/ID/SSN, address, mailing address (if different), and signature.
Section B – Type of placard requested (temporary, standard, permanent, renewal, replacement, or additional). Note the previous placard number if applicable. An optional donation to Opportunities for Ohioans with Disabilities (OOD) may be made.
Section C – Health care provider certification. The provider must confirm the applicant meets one or more of the seven qualifying disability criteria and state the expected duration of the disability (or that it is permanent).
Qualified health care providers include physicians, physician assistants, advanced practice registered nurses, optometrists (for visual impairments only), and chiropractors.

Types of Placards and Expiration
- Temporary – Disability expected to last 6 months or less.
- Standard – Disability expected to last more than 6 months (up to 10 years maximum).
- Permanent – Disability is permanent (no expiration date).
The expiration date is determined by the health care provider’s statement. Placards must be hung from the rear-view mirror when parked and removed while driving.
Important Rules, Display Requirements, and Penalties
The placard must be clearly visible from both the front and rear of the vehicle when hung from the rear-view mirror. Misuse of a disability placard is a misdemeanor under Ohio law.
Parking in an accessible space without a valid placard or accessible plates carries a fine of at least $250 and up to $500. Providing false information on the application or a prescription can result in criminal charges, fines, imprisonment, and permanent loss of placard privileges.
