Applicants must generally hold a valid Illinois driver’s license or state ID card. Military veterans should attach a copy of their DD214 when applicable.
Types of Disability Parking Placards Available
The Illinois Secretary of State issues four main types of placards:
- Meter-Exempt Permanent Placard (yellow and gray striped) — For permanent disabilities that also meet stricter criteria for free metered parking. Requires completion of Part 4 of Form VSD 62.
- Permanent Placard (blue) — For permanent disabilities. Allows parking in designated accessible spaces but does not exempt meter fees.
- Temporary Placard (red) — Valid for the duration specified by the medical professional, up to a maximum of six months.
- Organizational Placard (green) — Issued to organizations that transport persons with disabilities (requires a different form, VSD 816).
Only one placard is issued per eligible person. It may be used in any vehicle as long as the person with the disability is the driver or passenger and is entering or exiting the vehicle at the accessible space.

Fees and Related Costs
Disability parking placards themselves are free for eligible Illinois residents. Replacement placards (for lost, stolen, or damaged ones) require Form VSD 415 and cost $10.
For Persons with Disabilities license plates, you must pay the standard plate transfer fee (currently $29) plus the annual vehicle registration renewal fee. Disability plates can only be obtained after you have a permanent placard. Visit a local DMV with your permanent placard number and current plate or VIN information to apply for plates.
Important Rules for Using Disability Placards and Plates
- The placard or plates may only be used when the person with the disability is the driver or a passenger of the vehicle.
- Misuse or false application can result in revocation of the placard, a 12-month driver’s license suspension or revocation, and a fine of up to $1,000.
- Most other U.S. states and some foreign countries recognize Illinois disability placards and plates, but meter-exempt privileges may not apply elsewhere. Check local rules when traveling.
- If the authorized holder no longer needs the placard or passes away, contact the Persons with Disabilities Placard Unit at 800-252-8980 (option 4).
- Owners of disability license plates must still complete Form VSD 62 to obtain or renew a valid parking placard.
Meter-Exempt Parking Criteria
To qualify for free metered parking (yellow and gray striped placard), the applicant must have a valid Illinois driver’s license, an ambulatory disability listed in Part 3, and meet at least one of these additional criteria:
- Cannot manage, manipulate, or insert coins or obtain tickets due to lack of fine motor control in both hands
- Cannot reach above their head to a height of 42 inches from the ground
- Cannot approach a parking meter due to use of a wheelchair or other mobility device
- Cannot walk more than 20 feet due to a severe orthopedic, neurological, cardiovascular, or lung condition
- Missing a hand or arm or permanent loss of use of a hand or arm
- Under 18 years of age and incapable of driving
Economic need is not a factor for meter-exempt eligibility.
