NJ Form SP-41 Disability Plates & Placard Guide – New Jersey residents with qualifying permanent disabilities can obtain special wheelchair symbol license plates and/or a persons with a disability parking placard through the New Jersey Motor Vehicle Commission (MVC). The official application is Form SP-41, titled “Application for Vehicle License Plates and/or Placard for Persons with a Disability.”

What Is New Jersey Form SP-41?

Form SP-41 is the official New Jersey MVC application used to request:

One set of wheelchair symbol license plates
One persons with a disability parking placard
Both plates and a placard

Approved applicants also receive a Person with a Disability Identification Card. This ID card must be carried by the qualified individual whenever the plates or placard are used.

Who Qualifies for Disability Plates or a Placard in New Jersey?

Eligibility is limited to specific permanent medical conditions defined by New Jersey law. According to the New Jersey Motor Vehicle Commission, a person qualifies if they meet one or more of the following criteria:

Has lost the use of one or more limbs due to paralysis, amputation, or other permanent disability.
Is severely and permanently disabled and cannot walk without the use of or assistance from a brace, cane, crutch, another person, prosthetic device, wheelchair, or other assistive device.
Suffers from lung disease such that forced expiratory volume in one second (measured by spirometry) is less than one liter, or arterial oxygen tension is less than 60 mm/Hg on room air at rest, or uses portable oxygen.
Has a cardiac condition classified as Class III or Class IV under American Heart Association standards.
Is severely and permanently limited in the ability to walk because of an arthritic, neurological, or orthopedic condition, or cannot walk 200 feet without stopping to rest.
Has a permanent sight impairment of both eyes, as certified by the New Jersey Commission for the Blind (placard only).
Is a veteran certified by the U.S. Department of Veterans Affairs as permanently 100 percent disabled.

How to Apply Using Form SP-41?

Follow these steps to apply for permanent disability plates and/or a placard:

Download and complete Form SP-41.
Have a qualified medical practitioner complete and sign the medical certification section. The practitioner must also provide a prescription (or a letter on official letterhead if they cannot issue prescriptions) confirming the qualifying condition. The certification must be dated within 60 days of submission.
If applying for license plates, include a photocopy of the current vehicle registration.
Applicants without a New Jersey driver’s license or non-driver ID must meet the 6 Points of ID verification requirements and provide proof of residential address.
Submit the completed application either in person at a New Jersey Motor Vehicle Commission agency or by mail.

Who Can Certify the Medical Condition?

Only the following professionals may complete the medical certification on Form SP-41:

Physician
Podiatrist
Chiropractic physician
Physician assistant
Nurse practitioner

These practitioners must be licensed in New Jersey or a bordering state, or be a physician stationed at a military or naval installation in New Jersey who is licensed in any state. For 100% disabled veterans, a representative of the U.S. Department of Veterans Affairs may certify eligibility under the specific veteran category.

Fees for Disability Plates and Placards

There is no fee for permanent wheelchair symbol license plates or the persons with a disability placard when applying with Form SP-41. There is also no fee for recertification or replacement of a placard or identification card in most cases.

Note: Temporary placards (Form SP-68) are handled differently through municipal police departments and may involve a small fee.

Renewal and Recertification Requirements

Disability privileges in New Jersey are valid for three-year periods. Recertification of the medical condition is required every three years.

Wheelchair symbol license plates must still be renewed annually as part of the regular vehicle registration process, with medical recertification required every three years.
Placards require full medical recertification every three years using Form SP-41.
The medical certification must be dated within 60 days of the renewal submission.
Only original documents are accepted; photocopies are not permitted for recertification.

How to Use Disability Plates and Placards Correctly?

Proper use is strictly enforced:

The Person with a Disability Identification Card must be carried by the qualified individual at all times when using the plates or placard.
The placard must be hung from the rearview mirror only when the vehicle is parked in a designated disability parking space and must be removed when the vehicle is in motion.
The qualified person named on the ID card must be present in the vehicle (as driver or passenger).
Plates may be issued only for a vehicle owned, operated, or leased by the person with a disability or a family member who provides transportation.
Misuse or providing false information on the application is a fourth-degree crime under New Jersey law and can result in fines up to $10,000 and imprisonment of up to 18 months, plus revocation of privileges.

Replacing Lost, Stolen, or Damaged Documents

To replace a lost, stolen, or damaged placard, identification card, or plates:

Complete Form SP-41 and check the replacement section.
For lost items, attach a notarized statement of loss.
For stolen items, attach a police report.
For damaged items, return the damaged plate(s), placard, and/or ID card.

Replacements can be processed in person at an MVC agency or by mail to the Special Plate Unit. There is generally no fee for replacing a placard or identification card.

Temporary vs. Permanent Disability Privileges

Form SP-41 is used only for permanent conditions. Temporary placards (valid for six months, with one possible six-month renewal) require a different form (SP-68) and must be submitted to the municipal Chief of Police in the applicant’s town of residence, not the MVC.
