What Is Ohio Form BMV 1152?

Form BMV 1152 is the official “Application for BMV Fee Installment Plan.” It enables people whose driving privileges were previously suspended to enter a payment plan for reinstatement fees once all mandatory suspension periods have ended and other reinstatement requirements (except fee payment) are met.

The plan covers reinstatement fees and certain related fees payable to the registrar due to a suspension. Once approved and while payments remain current, the BMV generally records the driver’s status as valid, expired, or eligible to retest (as applicable).

Eligibility Requirements for the BMV Fee Installment Plan

To qualify, you must meet all of the following conditions (as stated on the official form and BMV website):

• You owe a minimum of $150.00 in reinstatement fees.
• You have met all other reinstatement requirements (for example, providing proof of insurance, completing any required remedial course, etc.).
• You are not currently enrolled in a court-ordered fee payment plan. A court-ordered plan takes precedence and must be terminated by the court before you can apply for the BMV plan.
• Your mandatory suspension periods have ended, and you have no pending suspensions that will take effect.
• You can provide current proof of financial responsibility (insurance).

Eligibility is determined by the BMV after it receives your complete application. Meeting the criteria does not guarantee automatic approval.

How to Complete Form BMV 1152

The form is straightforward. Fill it out legibly (illegible applications cause delays). Required information includes:

• Last name, first name, and middle name
• Driver license number
• Date of birth
• Current street address, city, state, ZIP code, and county

Sign and date the acknowledgment section. By signing, you confirm that you understand the terms, are applying voluntarily, and acknowledge that the plan involves predetermined fees paid in installments without interest and is not a consumer credit transaction.

Required Documents and Initial Payment

Submit the following together:

• Completed and signed Form BMV 1152
• Current proof of insurance (insurance card, declaration page, or other acceptable verification)
• Initial payment of $25.00 (applied to your reinstatement fees)

Important: Failure to include the $25.00 initial payment results in automatic denial of the application. If the application is denied for any reason, the $25.00 is still applied to outstanding fees and is non-refundable.

How and Where to Submit Form BMV 1152

You may submit the application, proof of insurance, and $25.00 payment by mail or in person:

• By mail: Ohio BMV, Attn: Remittance/Points (or Revenue Management/Points), P.O. Box 16521, Columbus, OH 43216-6521
• In person: Any BMV Deputy Registrar license agency (locations listed on the official BMV website). A service fee applies at these locations.

What Happens After You Submit the Application

The BMV reviews your application and determines eligibility. You will receive written notice of approval or denial.

If approved, your license status can become valid, expired, or eligible to retest (depending on your situation) as long as you stay current on payments. If denied, the initial $25.00 payment is credited toward any reinstatement fees you owe.

Payment Requirements Once Approved

After approval:

• A minimum payment of $25.00 is due every 30 days, calculated from the date of your last payment.
• You may pay more than the minimum or pay more frequently, but the next due date is always 30 days from the most recent payment.
• The BMV does not send bills or payment reminders. It is your responsibility to track payments.

Accepted payment methods include:

• Online at www.bmv.ohio.gov or by phone at (844) 644-6268 (a $15.00 minimum may apply for some online/phone transactions, and a service fee may be charged)
• In person at any Deputy Registrar (cash, check, money order, or credit/debit card; service fees apply, plus an extra fee for card payments)
• By mail (check or money order only) to the appropriate BMV remittance address

Note that service fees at Deputy Registrar locations are separate from the installment amounts.

Rules, Cancellation, and Important Notes

The installment plan may be denied or cancelled if:

• Your driving privileges become suspended during the plan
• You fail to make the required minimum payments every 30 days
• Any other reason determined by the BMV

If the plan becomes inactive due to non-payment, making a qualifying payment can reactivate it once the payment is applied to your record. Court-ordered payment plans supersede the BMV plan.
