Ohio BMV 4507: Gratis Military License Plates Form – Ohio Form BMV 4507 is the official Registration Application for Gratis and Free Military License Plate(s) issued by the Ohio Department of Public Safety, Bureau of Motor Vehicles (BMV). This form allows eligible U.S. military veterans, service members, and certain family members to obtain specialty military license plates at no additional cost beyond standard registration fees (or completely free in qualifying cases).
The form, revised as of September 2025 (BMV 4507 9/25), is used primarily for:
- Veteran with Disabilities plates
- Congressional Medal of Honor plates
- Former Prisoner of War (POW) plates (and spouses of deceased former POWs)
- Military logo plates selected in lieu of (or in addition to) Veteran with Disabilities plates
- Free boat trailer plates in certain cases

Who Qualifies for Gratis Military License Plates in Ohio?
Eligibility is strictly limited and requires official documentation. The qualifying military member must be listed as an owner on the vehicle title.
Veteran with Disabilities
Prior military service members who meet either of the following:
- Have a service-connected disability rated or compensated at 100% by the U.S. Department of Veterans Affairs (VA), or
- Have received a monetary allowance from the VA toward the purchase of a motor vehicle due to service-connected disabilities
These plates also allow use of accessible parking spaces. Applicants may instead choose up to two military logo plates of any eligible variety in lieu of the standard Veteran with Disabilities plate.

Congressional Medal of Honor
Only recipients of the Congressional Medal of Honor may apply. Documentary evidence of the award is required. Available for passenger vehicles, non-commercial trucks, recreational vehicles, house vehicles, non-commercial trailers, and unconventional vehicles.
Former Prisoner of War (or Spouse of Deceased Former POW)
Members of the U.S. Armed Forces who were imprisoned by an enemy of the United States. Spouses of deceased former POWs may also qualify. Required documents include a DD-214 or honorable discharge plus one of the following:
- Documentary evidence of POW status from a U.S. Armed Forces branch
- Telegram advising the family of POW status
- Letter from the War Claims Commission or U.S. Department of Veterans Affairs
- For spouses: copy of the death certificate or marriage certificate
These applications must be submitted by mail only.

How to Apply Using Form BMV 4507?
Follow these official instructions carefully:
- Complete all sections of the form. A signature is required.
- If the vehicle has not been permanently plated, submit the original Certificate of Title or Memorandum of Title. For mail applications, you may have the title verified at a local Deputy Registrar office for an $8 fee instead of mailing the original.
- Submit Affidavit for Registration (BMV 5712) if the vehicle is a non-commercial truck, non-commercial trailer, or utility car without a current plate.
- Complete any required E-Check emissions test (gasoline vehicles 6–25 years old or hybrid vehicles 7–25 years old in Cuyahoga, Geauga, Lake, Lorain, Medina, Portage, or Summit counties). Contact (800) CAR-TEST or visit ohioecheck.org.
- Attach all required supporting documents based on the plate type requested.
- Indicate the specific military logo plate (if selecting one in lieu of Veteran with Disabilities) on page 2 of the form.

In Person (available for most plate types except Medal of Honor and POW):
Visit a local Ohio BMV Deputy Registrar license agency with completed Form BMV 4507 and all supporting documents. Plates are typically mailed in 7–10 business days.

Personalized and Initial Reserve Options
Personalized ($50 annual fee) or Initial Reserve ($25 annual fee) formats are available only for military logo plates issued in lieu of Veteran with Disabilities plates. Important Notes and Restrictions
- The quantity of free plates is limited (typically up to two for qualifying disabled veterans).
- Most gratis military plates are issued without validation stickers (except military logo plates).
- Military logo plates selected in lieu of Veteran with Disabilities plates expire and must be renewed.
- False information on the application is subject to prosecution under Ohio Revised Code 2913.42.
- Proof of insurance or other financial responsibility is required to operate any vehicle in Ohio.
