Under 75 Pa.C.S. § 1553, the OLL authorizes operation of designated vehicles only when necessary for the driver’s occupation, work, trade, medical treatment, or study.

The OLL does not restore full driving privileges. It limits driving to approved routes, times, and purposes listed on the accompanying DL-15A affidavit. Drivers must carry both the photo OLL and the completed DL-15A at all times while operating a vehicle.

Who Is Eligible for a Pennsylvania OLL?

Eligibility depends on the reason for suspension and your driving record. Key requirements include:

Your driving privilege must be suspended (not revoked, canceled, or recalled).
You must have been previously licensed in Pennsylvania or another state.
You cannot have received an OLL within the past five years.
All outstanding fines, costs, citations, cancellations, and restoration fees must be paid.
Your restoration requirements letter should list only term suspension(s).

Suspensions related to DUI (75 Pa.C.S. § 3802 or former § 3731), chemical test refusal (§ 1547), or certain serious offenses generally make you ineligible until the full suspension term is served. Examples of ineligible violations (until fully served) include:

Driving under the influence or ARD for DUI
Refusal to submit to chemical testing
Homicide by vehicle
Fleeing a police officer
Racing on highways
Passing a school bus

What Is PennDOT Form DL-15-15A?

Form DL-15-15A combines detailed instructions with the official Occupational Limited License Petition (DL-15) and the Limited License Affidavit (DL-15A). The current version is marked DL-15 (7-25). It requires:

Driver personal information
Vehicle information (year, make, model, plate, state) and corresponding insurance details
A detailed written explanation of why limited driving is essential
Employer, school, or medical provider information and driving schedules
Signatures and acknowledgments

The DL-15A portion must be completed and carried with the issued OLL. You may begin filling out the affidavit while waiting for your camera card.

How to Complete Form DL-15-15A Step by Step?

Follow the instructions printed on the form carefully. Incomplete or inaccurate petitions are often denied or delayed.

Section A – Driver Information: Enter your full name, license number, date of birth, current address, telephone number (daytime hours 8:00 a.m.–4:30 p.m.), and any name or address changes. Provide supporting documents for name changes if applicable.
Section B – Name Change (if needed): Check the appropriate box and attach required notarized or certified documents.
Section C – Vehicle Information: List every vehicle you intend to drive under the OLL, including year, make, model, license plate, and registration state. Attach additional sheets if necessary.
Vehicle Insurance Information: Provide insurance company name, policy number, effective date, and expiration date for each vehicle. Copies of proof of insurance (not originals) must be included.
Section D – Explanation of Need: Write a clear, detailed explanation of why driving is essential for your occupation, work, trade, medical treatment, or study. Include typical days and hours. Attach extra pages if needed.
Employer / School / Medical Sections: Complete the appropriate sections (W1 for primary job, additional for secondary jobs, S1 for school, T for medical treatment). Include contact information and have the employer, school administrator, or medical provider sign the acknowledgment.
Driving Schedule Charts: Use the provided charts with destination codes (H = Home, W1 = Primary Job, etc.). For non-routine schedules (sales, delivery, etc.), describe the territory and hours in detail.
Acknowledgments and Signature: Review all statements, check optional contribution boxes if desired, sign in ink, and date the form. Misstatements are a third-degree misdemeanor.

The petition cannot be submitted in person at Driver License Centers. It must be mailed.

Required Fees and Supporting Documents

Include a check or money order payable to PennDOT for the following (combine into one payment if possible):

$88.00 non-refundable OLL Petition fee (required)
Restoration fee (amount shown on your restoration requirements letter; call 717-412-5300 if unsure)
License renewal fee and Form DL-143 if your license is expired or will expire during the suspension (Class C renewal is currently $39.50)
Optional: $3.00 Organ Donation Awareness Trust Fund or $5.00 Veterans’ Trust Fund contribution

Also enclose:

Copies of proof of insurance for every listed vehicle
Proof of self-employment (e.g., 1099) if applicable
Any additional supporting letters from employer, school, or medical provider

All fines and court costs must already be paid.

How and Where to Submit the Petition?

Mail the completed Form DL-15-15A, payment, and supporting documents by certified mail to:

To maintain continuous driving privileges, submit the petition at least 20 days before your suspension begins. PennDOT typically responds in writing within 20 days of receipt (plus mailing time). If approved, you will receive an OLL camera card. Take it to a Photo License Center to obtain your photo OLL.

If the petition is approved close to the suspension start date, PennDOT may issue an interim license and delay the suspension start by up to 15 days.

After Approval: Using Your OLL and DL-15A

Once you receive the photo OLL:

Carry the photo OLL and the completed DL-15A affidavit together at all times while driving.
Drive only the designated vehicles, during the approved times and for the approved purposes.
Update the DL-15A yourself if you change vehicles, address, or schedule (no need to notify PennDOT for vehicle changes alone).
For name or address changes on the OLL itself, submit Form DL-80OP along with an updated DL-15A.

The OLL remains valid for the length of your suspension term. After restoration, PennDOT will return your regular driver’s license. You may receive only one OLL every five years. A new violation that triggers cancellation, suspension, or revocation will result in recall of the OLL.

Common Reasons for Denial and Next Steps

Petitions are commonly denied for ineligible violations, incomplete information, unpaid fees, or failure to demonstrate occupational, medical, or educational necessity. Hardship or extraordinary medical circumstances alone do not qualify.
