How to Complete Pennsylvania Form MV-350A?
Sections A through D of Form MV-350A must be fully completed. Follow these steps carefully:
Section A – Dealer Information
Enter the Dealer/Business Partner Identification Number, exact business name as shown on the dealer authorization card, principal place of business street address, city, county, state, ZIP code, and business phone number.
Section B – Application for Additional Registration Plates
Select the plate type(s) needed and calculate fees based on the number of months remaining until the current plates expire (including the month of application) multiplied by the monthly fee. Indicate the number of plates requested. Answer the questions regarding the number of vehicle salespersons employed, projected monthly sales, plates for personal use, loaner vehicles, and driving school instruction.
Section C – Insurance Information
Provide the insurance company name, policy number, effective date, and expiration date.
Section D – Certification
The owner (and co-owner if applicable) must sign and print their name, certifying under penalty of law that all information is true and correct. False statements can result in a misdemeanor of the third degree.
