When Do You Need to Use Form MV-41?

Section A – Vehicle Owner Information

Section B – Correction or Verification of VIN or Application for State Replacement VIN Plate
Record the correct VIN. Attach a VIN tracing or have an authorized notary public employed by a full agent or motor vehicle dealer, or a certified inspection mechanic, complete the verification. For replacement plates, a police officer must complete the verification section with signature, badge number, department name, and telephone number.
Section C – Vehicle Data That Requires Correction
Check only the blocks that need correction (body type, make, year, seating capacity, unladen weight, registered gross vehicle weight, registered gross combination weight, odometer, number of axles, fuel type, etc.) and provide the correct information.
Section D – Verification of Weight Rating
Must be completed by an authorized inspection mechanic only.
Section E – Certification
The owner (and co-owner, if applicable) must sign and date the form, certifying that all information is true and correct under penalty of law.
Who Can Verify a VIN on Form MV-41?
VIN verification may be performed by:
- Authorized Pennsylvania vehicle safety inspection mechanics
- Notaries public employed by a full agent or registered motor vehicle dealer
Agents, inspectors, and law enforcement personnel are prohibited from verifying the VIN of their own personal vehicles. A photograph of the VIN plate is acceptable if it is signed and dated by a certified inspection mechanic.
If the vehicle is physically located outside Pennsylvania, the VIN must be visually verified by a police officer, a mechanic employed by a motor vehicle dealer, or an Armed Forces installation provost marshal or motor transportation officer using Form MV-41.
